Basic Requirements

To qualify for ReviVe Credit, applicants must:

• ✅ Be a Filipino citizen or legal resident of the Philippines

• ✅ Be at least 21 years old (not over 65 at loan maturity)

• ✅ Present valid IDs – at least one government-issued ID plus company ID

• ✅ Provide proof of billing (e.g., Meralco, Maynilad, or other utility bill)

• ✅ Show proof of income (latest payslip, COE, or ITR)

• ✅ Submit latest 3 months bank statements

How to Apply for ReviVe Credit

1. Prepare All of the Requirements

• Download the ReviVe Credit Application Form from our website or request a copy from our clinic.

• Complete all fields and attach the required documents.

• Submit your accomplished form and scanned requirements via email: reviveph.credit@gmail.com

Applying for ReviVe Credit is simple and hassle-free.

2. Application Review & Evaluation

Our team will review your documents and conduct a quick credit evaluation to determine your eligibility.

3. Pay Your Downpayment

Once approved, you’ll need to pay 20%–30% downpayment of your chosen treatment or package.

4. Loan Approval & Agreement Signing

After your downpayment is settled, you will sign the loan agreement detailing your payment terms, monthly interest, and schedule.

5. Enjoy Your Treatment or Products

You can now proceed with your treatment or purchase and pay the remaining balance in easy monthly installments (up to 12 months).
